Position Overview

We are seeking an experienced and compassionate Behavioral Health Case Manager to join our Clinical Health Services team. In this role, you will conduct comprehensive assessments, coordinate care, support members during behavioral health or medical crises, and ensure members receive high‑quality, clinically appropriate services. You will apply advanced clinical knowledge, collaborate across multidisciplinary teams, and empower members to make informed decisions about their health.

Key Responsibilities

Member Assessment & Crisis Support

  • Conduct detailed clinical assessments to determine member needs, eligibility, and appropriate care pathways.
  • Evaluate benefit plans and identify applicable internal and external resources.
  • Use clinical judgment to identify risk factors and address complex behavioral or medical indicators.
  • Provide crisis intervention and ensure members receive timely clinical assessments and appropriate treatment.
  • Perform follow-up outreach to ensure ongoing engagement in recommended services.

Care Coordination & Quality Enhancement

  • Apply and interpret clinical guidelines, criteria, and policies to ensure appropriate care and benefits administration.
  • Collaborate with Medical Directors, supervisors, and multidisciplinary teams to address barriers and enhance outcomes.
  • Identify and escalate potential quality-of-care issues.
  • Communicate effectively with medical and behavioral health providers to influence evidence‑based care decisions.
  • Use motivational interviewing and coaching techniques to encourage healthy lifestyle changes and member engagement.
  • Review utilization, self-reported, and clinical data to identify and address comprehensive member needs.

Monitoring, Documentation & Care Planning

  • Partner with members and care teams to develop personalized care plans aligned with member goals.
  • Monitor progress and adjust plans as needed to support successful outcomes.
  • Maintain thorough, accurate documentation in accordance with organizational, regulatory, and accreditation standards.
  • Adhere to case management and quality management processes to ensure consistent and compliant service delivery.

Required Qualifications

  • Unencumbered Behavioral Health clinical license in the state of practice (e.g., LCSW, LCPC, LMFT, etc.).
  • Master’s degree in Behavioral Health, Mental Health, or a related field.

Preferred Qualifications

  • 3–5 years of post‑master’s direct clinical experience in a hospital, outpatient clinic, or alternative care setting.
  • Case management or discharge planning experience.
  • Experience in managed care or utilization review.
  • Crisis intervention training and experience.

Pay Range

The typical pay range for this role is:

$66,575.00 - $142,576.00

This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.
