Position Summary
The Senior Manager for Care Model Management is responsible for collaborating with the Executive Director in strategically developing, implementing, and continuously optimizing care delivery models that improve patient outcomes. This role acts as a key leader within the organization, working collaboratively across functions to ensure that care models are based on evidence, sustainable, and aligned with regulatory standards and the organization's mission.

  • Care Model Development:
    • Create, assess, and continually improve care models to boost patient experience, quality, and value. Direct the evaluation of existing care delivery systems and pinpoint areas for innovation and enhancement.
  • Stakeholder Collaboration:
    • Collaborate closely with clinical, operational, administrative, and executive leaders to ensure care models align with enterprise strategy and workflows.
    • Support and influence multidisciplinary teams through effective change management, driving adoption, accountability, and sustained performance across the organization.
  • Planning & Execution

·Own and maintain the integrated care model master plan, including roadmap, milestones, dependencies, and key decision points.

·Ensure individual workstream plans roll up cleanly to overall program milestones and objectives. Validate progress based on achievement of outcomes and value realization—not solely task completion—using data‑driven insights to guide execution and course correction.

·Risk, Issues & Escalation Management:

·Own cross‑workstream risks, issues, and dependencies, assessing cumulative impact to timelines, value realization, and organizational commitments.

·Proactively lead escalation and mitigation strategies in partnership with Care Model leadership and Executive leadership to ensure timely resolution and informed decision‑making.

·Performance Measurement & Value Realization:

·Define, track, and monitor key performance indicators (KPIs) to evaluate care model effectiveness and value delivery. Use analytics and insights to inform strategic decisions, continuous improvement, and long‑term sustainability.

·Reporting & Executive Communication:

  • Produce integrated, program‑level reporting for governance bodies and executive audiences. Synthesize inputs from multiple workstreams into a single, cohesive narrative that clearly frames progress, risks, decisions, tradeoffs, and leadership asks.
  • Ensure Transparency and clarity to support timely executive action.

Requirements:

  • Minimum of 7 years’ experience in healthcare operations, care model design, or clinical management, with at least 3 years in a leadership role.
  • Proven track record of leading cross-functional teams and managing intricate projects.
  • Strong understanding of healthcare delivery systems, regulatory requirements, and value-based care principles.
  • Excellent communication, organizational, and analytical skills

Education

  • Bachelor’s degree in healthcare administration, nursing, public health, or a related area.

Pay Range

The typical pay range for this role is:

$67,900.00 - $182,549.00


This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above. This position also includes an award target in the company’s equity award program.
