The Virtual Primary Care Nurse Practitioner works in collaboration with a dedicated care team to provide evidence-based, primary care inclusive of accurate assessment, diagnosis, treatment, management of health problems, health counseling, and disposition planning for patients 18 years of age and older. This full-time clinical position is 100% remote. The care team will work with NP daily to drive better outcomes for the entire panel. Ideal candidate is FNP licensed in one or more states in the Western U.S.

What you'll do:

Patient-Centered Quality and Safety

  • Demonstrate knowledge of etiologies, risk factors, pathophysiology, presentation, and epidemiology of medical conditions relevant to primary, acute, chronic, and transitional care patients

  • Perform history and physical assessments of patients that are accurate, complete, concise, relevant and appropriate for age, gender and clinical issue

  • Evaluate and treat patients' medical needs with safe and high-quality care

  • Counsel and educate patients and families

  • Document all patient care within an electronic health record in line with current policies and procedures

  • Work as a member of the care team providing care and coordination of our patients with internal and external colleagues ensuring the highest level of care is provided for all patients and at all time

  • Provide patient centered care that is compassionate, appropriate and effective for the promotion of health, prevention of illness and treatment of disease

  • Referral to appropriate Specialists and patient care components as appropriate

  • Possess an awareness and a working understanding of quality measurement in the management of chronic disease conditions

  • Document patients that have expressed a desire to join care team panel, using approved EMR workflows

  • Participate in/lead practice committees (e.g. Epic Optimization, Care Pathways, etc.)

REQUIRED QUALIFICATIONS & ABILITIES

  • Active Advance Practice Registered Nurse, Nurse Practitioner license in good standing

  • Current National Board Certification, Family Practice

  • 5+ years of recent Primary Care experience as a Nurse Practitioner in family medicine or primary care

  • Effective verbal, written, and electronic communication skills

  • Outstanding organizational skills and ability to multi-task

  • Initiative, problem solving ability, adaptability and flexibility

  • Ability to work remotely in a care team culture

  • Is proficient with information management and technology

  • Obtain, and maintain in good standing, multi-state licensures

  • Take after-hours and/or overnight call to support patient care

WORKING ENVIRONMENT

  • Virtual Care Team members must meet minimum requirements for remote care delivery, including broadband connectivity, a quiet setting with a neutral background to conduct visits from, and the ability to uphold patient privacy per CVSH guidelines

  • While performing the duties of the job, the employee is regularly required to interact with patients remotely, sit, write, operate the computer and phone, speak intelligibly, and hear patient responses

  • Specific vision abilities include the ability to view and read a computer screen and other electronic devices

EDUCATION

Completion of master’s degree level Family Nurse Practitioner program with current National Board Certification and State of Employment license to practice in the Advanced Practice Nurse role required

Pay Range

The typical pay range for this role is:

$87,035.00 - $224,952.00

This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors.
