Remote Case Manager RN (Nationwide)

100% Remote | Full-Time | No Travel

Are you a compassionate, detail-driven RN who thrives in a fast-paced, patient-focused environment? Join our team as a Remote Case Manager RN and play a critical role in guiding members toward better health outcomes through personalized care coordination.

What You’ll Do

• Conduct comprehensive clinical assessments and develop individualized care plans
• Coordinate inpatient reviews and discharge planning
• Advocate for members and connect them with appropriate services and resources
• Monitor care plans and adjust strategies to ensure optimal, cost-effective outcomes
• Use motivational interviewing and coaching to support behavior change and engagement
• Identify quality of care issues and contribute to program improvements
• Deliver exceptional member service in a remote environment

What You Bring - REQUIRED

• Active, unrestricted RN license (with ability to obtain multi-state licensure paid for by the company)
• 3+ years of acute care RN experience
• Strong clinical judgment and decision-making skills
• Excellent communication, organization, and multitasking abilities
• Proficiency in Microsoft Office tools

Preferred Qualifications

• Compact RN license
• Certified Case Manager (CCM)
• Case management or managed care experience
• Telephonic care or customer service experience
• Oncology care experience

Schedule

• Monday–Friday, 8:00 AM – 4:30 PM (local time)
• Occasional evening rotation (10:30 AM – 7:00 PM)
• Rotational evenings, weekends, and holidays based on department needs

Education

• Associate Degree in Nursing required (BSN preferred)

Pay Range

The typical pay range for this role is:

$54,095.00 - $116,760.00

This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.
