Position Summary

The Care Management Associate initiates contact with members within three days of discharge from a hospital or facility. This person ensures outreach is focused on discharge planning and scheduling follow-up visits with the member’s assigned Primary Care Physician (PCP).

In this role, you will:

• Apply critical thinking and clinical judgment to support the case management process.
• Collaborate with internal teams and external providers to facilitate appropriate healthcare outcomes.
• Use care management tools and resources to educate and support members.
• Promote and support the quality administration of healthcare services.
• Ensure that all actions align with organizational standards and regulatory requirements.

Required Qualifications

• 1-2 years of experience as a medical assistant, office assistant, or adjacent role.
• 1-2 years of experience in a call center environment.
• Must reside in Arizona.
  

Preferred Qualifications

• Experience working in managed care.
• Effective communication, telephonic, and organization skills.
• Familiarity with basic medical terminology and concepts used in care management.
• Strong customer service skills to coordinate service delivery, including attention to customers, sensitivity to issues, proactive identification, and resolution of issues to promote positive outcomes for members.
• Computer literacy to navigate through internal/external computer systems, including Microsoft Excel and Microsoft Word.

Education

• High school diploma or GED.

Pay Range

The typical pay range for this role is:

$18.50 - $35.29

This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors.
