Day-to-day responsibilities include verifying and dispensing prescriptions, reviewing medication orders for accuracy and clinical appropriateness, and monitoring for potential drug interactions and contraindications. The Pharmacist provides medication counseling to patients and caregivers, collaborates with physicians, nurses, and other health professionals on individualized treatment plans, and supports transitions of care through accurate medication reconciliation. The role also involves maintaining regulatory and safety standards, participating in quality improvement initiatives, contributing to pharmacy education and training activities, and using clinical judgment and technology systems to optimize medication therapy outcomes.Qualifications Candidates should possess strong clinical pharmacy skills, including medication therapy management, pharmacotherapy knowledge, and ability to interpret lab results and clinical guidelines.Candidates should possess skills in patient education and counseling, with the ability to communicate complex information clearly and support adherence to treatment plans.Candidates should possess proficiency with pharmacy information systems, electronic health records, and automated dispensing technologies to ensure accurate and efficient medication workflows.Candidates should possess skills in collaboration and teamwork, working effectively with interdisciplinary clinical teams and contributing to coordinated, patient-centered care.Candidates should possess attention to detail and strong organizational abilities to manage multiple medication orders, prioritize tasks, and maintain regulatory compliance.Doctor of Pharmacy (PharmD) or equivalent degree from an accredited program; active, unrestricted pharmacist license or eligibility for licensure in the State of Texas.Completion of a pharmacy residency (PGY-1, and PGY-2 if applicable) or equivalent clinical experience in an acute care or ambulatory care setting is preferred.Certification in relevant specialty areas (e.g., BCPS, BCACP, BCCCP) and experience in an academic or teaching hospital environment are beneficial.Commitment to culturally competent care, professional ethics, ongoing continuing education, and participation in
