Under direct supervision, responsible for ensuring the quality of medical records by verifying for completeness, accuracy, and proper entry into computer systems. Assembles and analyzes patient data for the purpose of improving patient care and controlling costs.

Vision - Fully insured Vision Care benefits are offered by Superior Vision Services. Two vision plan options that offer either standard or enhanced vision benefits.
Disability - Employees can enroll in the Disability Insurance which provides income if a non-work related illness or injury prevents you from working.
FSAs - Employees can enroll in flexible spending accounts (FSAs) to set aside money from earnings before taxes for qualifying dependent day care expenses or out-of-pocket health care expenses.
Retirement - Employees are eligible for either the Teacher Retirement System (TRS) or the Optional Retirement Plan (ORP). TRS is a defined benefit retirement plan which UT Health matches employee contributions. ORP is for eligible faculty staff employees. Voluntary retirement programs are also available to invest before- or after-tax dollars with the choice of five quality retirement plan providers.
Time Off - A generous leave program offers multiple paid leave options:
- Front-loaded Paid Time Off: 128 to 208 hours (16 to 26 days) of Paid Time Off based on years of service, given at the start of each fiscal year. PTO may be prorated in year one based on date of hire.
- Extended Illness Bank: 8 hours (1 day) accrued per month which can be used for illness or injury after one day of Paid Time Off is taken.
- Paid Family Leave: Up to 240 hours (6 weeks) to care for a spouse, child, or parent after 6 months of consecutive employment.
- Holidays: 12 set paid holidays each year.

EDUCATION:
- High school diploma or GED is required.
- Creates radiology image CD's to send out to requesting parties for surgical procedures, continuing care, and patient requests.
- Anonymizes radiology images prior to burning on the CD for research study sponsors.
- Performs quality review of anonymized images to ensure identifiable patient information has been removed from each image.
- Evaluates and reviews Epic MyChart Proxy requests. Uploads MyChart proxy application and legal documents to grant a legal guardian Epic access to health information for children under 18.
- Receives and processes patient uploaded Advanced Care Planning documents from Epic MyChart to include in the Electronic Medical Record.
- Serves as a LifeImage liaison for clinical staff and as a LifeImage Gatekeeper that reviews and manages radiology image nominations to the UTHP PACS.
- Manages the Transcription interface error queue and performs transcription training to providers. Serves as the liaison with the transcription vendor to coordinate updates and issue resolution.
- Manages and processes third party payor audit requests (BCBS, Community First, HEDIS, Humana, Medicaid, Medicare, WellMed, etc.)
- Trains UTHP clinical staff on new Health Information processes.
- Ensures specialization in coding diagnoses and procedures in patient records for research and special projects.
- Ensures records are properly coded/indexed into the appropriate management system(s).
- Assists HIM leadership with special projects as needed.
- Ensures specialization in coding diagnoses and procedures in patient records for research and special projects.
- Conducts quality checks on order entry and/or clinical documentation in order to instruct/train end users in the correct method of entry/selection/linking orders to appropriate document levels.
- Coordinates with physicians and patients to complete open encounters or records containing deficiencies.
- May conduct reviews of overdue results report and retrieve results from the appropriate facility.
- Provides coverage for release of information.
- Keeps all patient information confidential and abides with Privacy and Security regulations.
- Performs all other duties as assigned.
